Voorwaarden van dienstverlening

Geachte patiënt,

De organisatie van een artsenpraktijk is een complex gegeven. Onverwachte afwezigheden van patiënten leiden tot desorganisatie, terwijl het tijdig doorgeven van annulaties plaats vrijmaakt voor patiënten die een dringende afspraak vragen. In het belang van uzelf en van de praktijk gelden daarom de volgende voorwaarden van dienstverlening.

Voor uw afspraak werd voldoende tijd uitgetrokken voor een correcte en volledige diagnose of behandeling. U kunt een afspraak kosteloos annuleren op voorwaarde dat dit minimum 48u voor de afspraak gebeurt. Indien de afspraak niet kan doorgaan door uw afwezigheid of laattijdige aankomst is een vergoeding verschuldigd van 50 euro per afspraak voor een gewone consultatie, 100 euro per afspraak voor een laserbehandeling, 400 euro voor een operatieve behandeling. Kan de afspraak niet doorgaan door mijn verantwoordelijkheid, zonder dat ik u 48u op voorhand verwittig, dan ben ik u een vergoeding verschuldigd van 25 euro per gewone consultatie en 50 euro voor een behandeling, tenzij u een vervangafspraak binnen de 14 dagen krijgt.

Het ereloon voor de verrichte prestaties moet in de artsenpraktijk kontant worden betaald, ofwel cash ofwel via bancontact. Indien dit uitzonderlijk niet mogelijk is moet het bedrag de dag zelf worden overgeschreven. Op laattijdige betalingen zijn intresten verschuldigd aan de wettelijke intrestvoet vanaf de dag van de prestatie, en is een inningsvergoeding verschuldigd van 50 euro per aanmaningsbrief.

Door het aanvaarden van mijn prestatie aanvaardt u meteen ook deze voorwaarden.

Dear patient,

The organisation of a medical surgery is a complex matter. Unexpected absences of patients lead to disorganisation, while the timely notice of absence allows us to provide an appointment for a patient in need of an urgent appointment. Both in your own interest and that of the practice, the following conditions of service apply.

Enough time was taken for your consultation to allow for a correct and complete diagnosis or treatment. You can cancel an appointment provided the cancellation takes place at least 48 hours before your appointment. If the appointment cannot take place due to your absence or late arrival, a fee of 50 euros per appointment will be charged in case of an ordinary consultation , 100 euros for an appointment with laser treatment, 400 euros for a surgical treatment.

The fee for the services provided must be made with immediate payment at the medical surgery, either cash or by Bancontact. If not possible, as an exception, the amount must be transferred on the day itself. Interest is due for late payment at the legal interest rate, and this from the day of the medical performance, including a collection fee of 50 euros per formal notice.

Accepting my medical performance includes your immediate acceptance of these conditions.

Conditions de la prestation

Cher patient,

L’organisation d’un cabinet médical est une donnée complexe. Les absences inattendues des patients mènent à la désorganisation, tandis que la transmission rapide des annulations libère la place aux patients qui ont besoin d’un rendez-vous urgent. Dans votre intérêt et celui du cabinet, les conditions de prestation suivantes sont applicables.

Pour votre rendez-vous, on a alloué suffisamment de temps pour un diagnostic ou un traitement correct et complet. Vous pouvez annuler un rendez-vous à condition que cela soit fait au moins 48 heures avant le rendez-vous. Si le rendez-vous ne peut pas avoir lieu en raison de votre absence ou arrivée tardive, une indemnité de 50 euros est due pour un rendez-vous avec consultation normale, 100 euros pour un rendez-vous avec traitement au laser, 400 euros pour un traitement chirurgical.

Les honoraires pour les prestations effectuées doivent être payés au cabinet du médecin au comptant, soit en espèces ou par Bancontact. Si cela est exceptionnellement impossible, le montant doit être transféré par virement le jour même. Les retards de paiement sont soumis à des intérêts au taux d’intérêt légal à compter de la date de la prestation et une indemnité de 50 € est due par lettre de mise en demeure.

En acceptant ma prestation, vous acceptez également en même temps les conditions susmentionnées.
